THE 50-STATE RULES TABLE

This is the part nothing else on the shelf has. Twelve states prescribe their own statutory waiver wording, and a generic form can be void in those states. The State Rules tab covers all 50 states plus DC — statute cite, whether a form is mandated, notarization, whether advance waivers are enforceable, and the one practical thing to know per state.
